Insurance

Four Rivers Therapy and Psychological Services does not currently accept insurance; however, you may find that services are covered through your out-of-network benefits. A description of the services including dates and procedural codes will be provided upon request that you may submit to insurance providers for reimbursement. It is recommended that you contact your insurance company prior to your initial appointment so that you are aware of your coverage. Four Rivers Therapy and Psychological Services will not bill an insurance company directly or negotiate fees. Reimbursement through insurance is not guaranteed.

Questions to ask your insurance provider:

Do I have out of network benefits to see a licensed psychologist?

If so, what percentage of the session fee do you cover?

What is the deductible, and how much of the deductible have I met?

Do any exclusions apply? Do you only cover certain diagnoses or tests?

Do I need pre-authorization?

How do I access the form(s) needed to submit a request for reimbursement?

How many units of each CPT code will you reimburse?

Common procedure (CPT) codes:

90791 (60-90 minutes) = Intake appointment with patient present

90834 (45 minutes) = Individual Psychotherapy

90837 (60 minutes) = Individual Psychotherapy

90846 (60 minutes) = Family Therapy without client

90847 (60 minutes) = Family Therapy with client

96130, 96131, 96132, 96133, 96136, 96137= Psychological Testing

Fees

Full payment is made to Four Rivers Therapy and Psychological Services at the time of service. Four Rivers Therapy and Psychological Services accepts cash, checks, credit cards, and Health Savings Account credit cards/checks.

Therapy

Initial Intake Session (90 minutes): $225

Therapy Session (45 minutes): $160

Therapy Session (60 minutes): $175

Family Therapy (60 minutes): $200

Psychological Evaluations

Testing prices vary based on individual need and may range from $1500 to $2900 and will be discussed prior to service. Please note that the fee includes the intake appointment, testing materials, administration, scoring, interpretation, report writing, reviewing records, time spent contacting other professionals, the feedback session, the comprehensive report, and consultations. Payment is typically split into three payments: there is a $225 fee due on the day of the intake appointment, half of the remaining fee will be required at the time of the assessment, and the remaining balance will be due at the time of the feedback session.

Special Education/School Consultation

Service Fee: $175 per hour

Travel Fee: $87.50 per hour of travel (prorated based on total travel time)

Good Faith Estimate

You have the right to receive a "Good Faith Estimate" explaining rates for various services offered by your therapist. Upon scheduling services at Four Rivers Therapy and Psychological Services, you will receive written notice of your rights under the No Surprises Act, as well as a Good Faith Estimate of your estimated rates for services.

You have the right to receive your Good Faith Estimate in writing at least 1 business day before your scheduled healthcare service or item. If a service is scheduled at least 10 days in advance, the Good Faith Estimate must be provided within 3 business days of scheduling. You have the right to dispute a bill that substantially exceeds your Good Faith Estimate ($400 or above).

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 24 hours in advance. Since your appointment time is reserved specifically for you, 24 hours notice of a cancellation is required to avoid being charged a fee of $100 per missed session. This fee will also be applied for no-show appointments. Occasional exceptions occur if it is agreed that a late cancellation was unavoidable.
